Facial bone surgery is planned from medical imaging, not from a photograph. The cheekbone, the jaw, the chin and the two jaws each change a different part of the structure — and each can be pushed past the point where the face still reads as the same person.

A difference in experience is a difference in judgement.

The difference between a natural result and an artificial one starts at the balance point. When contouring misses it, the face either reads as off or has been changed further than it could carry.

Size and shape have to agree with the whole impression of a person. Correct the weakness, but never at the cost of the strength.

Finding that person's own balance is the core of the operation — and the reason an experienced eye is worth more here than anywhere else on the face.

I am the surgeon whose operating time never gets shorter.

Normally, when you repeat the same work, you get faster at it. My operating time simply will not come down. I want today's operation to be better than yesterday's, the afternoon's better than the morning's. The more I do it, the more I want to raise it, and the more things keep appearing to me that still need doing.

Years ago, while I was still building up, I took pride in operating quickly. Now I cannot do it quickly. There is no reason not to do it better when I can.

    The surgeon who operates is named on the record, and the operating room is under CCTV. Neither is unusual to ask for; both are easier to promise than to install.

    Malignant hyperthermia is rare and fast. The antidote is kept in the building, and the anaesthesia team is employed here rather than called in per case.

    Sagging is one of the known side effects of contouring, and a scheduled check-and-care programme is how part of it is headed off. The reviews are booked, not offered.

    Difficult cases are not decided alone. With surgeons across several fields on staff, a second reading costs a corridor rather than a referral.

The value of experience is not in seeing much, but in seeing wisely.
