Breast Augmentation Korea: Implant, Fat Transfer and Recovery

Most people searching breast augmentation Korea are not asking whether implants exist. They are asking who will be holding the instruments, how many days they will be sitting in a hotel room in Gangnam, and when they can fly. I am Dr Jaebeom Seo, and at Banobagi those are the three questions that take up most of a first consultation, long before anyone talks about size.

Search results call it a Korean boob job, a boob job in Korea, or breast implants Korea. Clinically there are two routes: an implant placed in a chosen layer of the chest, or fat transfer breast augmentation using your own tissue. They suit different bodies and different goals, and they do not have the same recovery.

Breast augmentation Korea: what the operation actually involves

Breast augmentation adds volume with an implant. Two decisions shape the result far more than the number on the implant box: where the incision goes, and which layer the implant sits in. Both are settled with you at the consultation, based on your breast tissue and the shape you are after.

The incision is placed in one of three positions, transaxillary (underarm), periareolar (at the areolar border) or inframammary (in the breast fold). The placement layer is then chosen from three options:

  • Under the muscle (submuscular): the implant sits below the chest muscle. Low chance of feeling the implant, natural breast shape, and effective when there is little breast volume to start with.
  • Over the muscle (subfascial): the implant is inserted between the chest muscle and the glandular tissue. Effective for breasts that already have some volume, and for correcting sagging and side-set breasts.
  • Dual plane: the upper part of the implant sits under the pectoral muscle and the lower half under the breast gland. Effective for thin skin.

Three things are standard in our theatre for this operation. A Full HD Endoscope Dual Vision system is used during dissection, which limits damage to the nerves and surrounding tissue and reduces bleeding and pain, so recovery is faster. The Keller Funnel keeps the implant from touching the skin, which reduces the risk of infection. And Dermabond closes the incision, so there are no stitches to remove afterwards. Implant or fat transfer breast augmentation: which route fits?

Fat transfer takes fat from a donor area such as the abdomen, thighs or flanks by liposuction, purifies it, and injects it into the breast in small amounts across different tissue layers. It uses no synthetic material and slims the donor site at the same time, but the volume it can add in one session is limited, and part of the graft is reabsorbed.

If you want a larger change than fat alone can give, a hybrid plan, implant plus fat grafting, is the option we discuss. Results differ between individuals, and which of the three routes we recommend depends on your tissue thickness and chest wall, not on preference alone.

Who it suits, and who should think about a lift first

The patients I see for augmentation usually recognise themselves in this list:

  • Breasts that feel too small, or that lost volume after pregnancy or weight change
  • One breast noticeably smaller than the other, with visible asymmetry
  • Little volume relative to your frame, or a silhouette that does not sit well under clothing
  • A wish for a natural feel and shape rather than maximum projection
  • An adult in good general health

Sagging is the fork in the road. If the breasts are relatively small and the ptosis is mild, the nipples less than 1-2cm above the inframammary fold, an implant alone can correct it. If the nipples point downward or sit below the breast crease, an implant adds volume to a breast that is still low, and a breast lift is the part that changes position. Augmentation with a lift is the combination we plan most often; it restores volume and corrects sagging in one operation and one recovery. Surgery day, stitches and the first three months

Augmentation takes about an hour under general anaesthesia, managed by anaesthesiologists employed by the clinic rather than booked in for the case. Many patients are discharged the same day; some stay one night.

What the weeks look like:

  1. Days 1-3. Swelling, tightness and bruising peak within 48-72 hours. Sleep on your back with the upper body elevated, and keep your arms below shoulder height. Prescribed pain medication and antibiotics are part of the plan.
  2. Days 5-7. Drains, if placed, are removed at the first post-operative visit. The surgeon checks incision healing, swelling and implant position. Light walking and short outings are usually comfortable.
  3. Weeks 2-4. Bruising fades. Most patients are back at desk work around week two and light lower-body exercise after two to three weeks. Keep wearing the compression bra as directed; avoid chest exercise, heavy lifting and sleeping on your side or front.
  4. Months 3-6. Residual swelling resolves and the implants settle into position. Incision lines fade from red towards skin tone.

Recovery speed differs between individuals, and the dates above are the shape of a normal course, not a promise about yours.

How long to stay in Seoul, and what to do before you fly

Plan on at least 10-14 days in Seoul for breast surgery. That covers the in-person consultation and examination, the operation, the swelling and dressing checks, and the post-operative visit for drain or stitch removal before you board a flight. Do not book the return leg before your surgeon has confirmed the plan.

Preparation that actually changes your surgery date if it is missed:

  • Stop aspirin, ibuprofen, fish oil, vitamin E and herbal supplements 1-2 weeks before surgery, they increase bleeding risk
  • Stop weight-loss medications such as Wegovy or Mounjaro at least two weeks before, and blood thinners at least one week before, after speaking to your prescribing doctor
  • Avoid alcohol and smoking for at least 1-2 weeks before surgery
  • Stop eating and drinking 6-8 hours before your scheduled time, including water and sweets. Surgery cannot go ahead if fasting is not followed
  • Pack front-button or front-zip clothing, and bring your passport and a list of your current medications

If you already have implants: revision and what we look for

A large share of the breast consultations I take are second operations. The reasons repeat: capsular contracture, a ruptured implant, asymmetry, implants sitting too close to the midline so the cleavage separation is lost, or a size and feel you were never happy with. Before planning anything, we work out why the first result turned out that way, pocket over-dissection, an implant too wide for the ribcage, or placement in the wrong layer all need different corrections.

  • Asymmetry is corrected after examining its degree, the position of the implants and the thickness of the chest muscles
  • A ruptured implant is removed completely and replaced
  • Thickened capsule tissue is removed, and implant position or sunken tissue is adjusted
  • Implants are exchanged for a size that fits after an extensive consultation
  • Scar care is doubled up for revision cases, carried out together with Banobagi dermatologists

Why we quote from your photos instead of publishing a price list

Banobagi does not publish a fixed price list, and that is a clinical decision rather than a way of avoiding the question. Two patients asking for the same thing can need different operations: one an implant in a dual plane, the other an implant plus a lift, a third a fat transfer with liposuction of two donor areas. A single advertised number would be wrong for at least two of them.

What moves your quote: implant versus fat transfer versus a hybrid plan, the placement layer, whether a lift or reduction is combined, whether it is a revision, the anaesthesia and whether you stay a night. How to get the figure:

There are cases where we say no. When the breasts sag beyond mild ptosis, we do not recommend an implant on its own, because volume added under skin that has already dropped does not lift it. A lift, or a lift with augmentation, is the plan we discuss instead.

Can I have the consultation online before I book a flight?

Yes. An online session settles which procedure suits what you want to change, roughly how long you need in Korea, what recovery looks like week by week, the cost range and which surgeon would take the case. It is a consultation and a plan, not a diagnosis; measurements and the final surgical plan are confirmed in person.

Will I have visible scars?

The incision can be placed in the underarm, at the areolar border or in the breast fold, and Dermabond is used for augmentation so the stitches do not have to be removed. Do breast implants have to be replaced later?

Implants are checked at regular examinations, where the condition of the breasts and the implants is reviewed and any tumour that is found can be removed. How soon can I go back to the gym?

Most patients return to desk work and social activities around week two and to light lower-body exercise after two to three weeks. Chest exercise, heavy lifting and sleeping on your side or front wait until your surgeon clears them. Recovery differs between individuals.

Can breast augmentation be combined with another procedure?

Augmentation with a lift is the combination we plan most often, because it restores volume and corrects sagging in one operation. Fat transfer already includes liposuction of the donor area. Combining procedures shortens total recovery time compared with separate trips, and we confirm what is sensible for you at the consultation.
